Job summary

Compassus is hiring a Home Health Licensed Practical Nurse to support patients in- home, helping them recover and stay at home with their families. You will educate patients on disease management and coordinate care with families and care teams.

The role emphasizes compassionate, home-based nursing with strong clinical skills and the ability to work independently in patients' homes.

Qualifications

• Graduate of an accredited school of Practical Nursing.
• Current state license as a Licensed Practical Nurse (LPN).
• Current driver’s license and CPR certification.
• Negative TB screen and hepatitis consent/declination.
• Two years of LPN experience; home health preferred.

Responsibilities

• Support patient recovery and the ability to stay at home, enabling quality time with loved ones.
• Educate patients and families on disease management and care plans.
