Qualifications of the Nurse:

• Med-surg background

• Must have a good standing license (LPN) in the state in which the clinician will practice.
• Current CPR certification.
• Must be willing to travel and visit patients in their homes.

Responsibilities of the Nurse:

• Provide one-to-one patient care in a home environment.
• Deliver specialized nursing care to medically complicated adult patients.
• Conduct patient assessments and coordinate care.
• Engage in health promotion, teaching, and training of patients and/or family members.
• Be available to work one weekend per month.
