Remote - Bilingual - Spanish + English - Licensed Practical Nurse - LPN - LVN

CareHarmony's Care Coordinators (LPN) (LVN) work comprehensively with providers to deliver value-based care management initiatives for their patients.

We are seeking an experienced Licensed Practical Nurse to work 100% remotely. The ideal candidate will have at least 3 years of direct patient-facing experience, thrive in a fast-paced environment, be self-motivated, attentive to detail, and committed to impacting patients' healthcare journeys.

You will have experience in identifying resources and coordinating needs for chronic care management patients.

Requirements include an active Multi-State/Compact License (LPN), proficiency in Microsoft Office, excellent communication skills, and experience with Epic (preferred). Additional licensures and bilingual skills are a plus.

Remote requirements include high-speed internet, a HIPAA-compliant workspace, and the ability to sit for extended periods, communicate clearly, and take thorough notes during calls.

Benefits include health insurance (medical, dental, vision), paid holidays, PTO, and sick leave. Compensation ranges from $22/hr to $28/hr, with opportunities for overtime.
