Job summary

Compassus is seeking a Home Health Licensed Practical Nurse (LPN) to support patient recovery at home and help families achieve health goals in a compassionate, community-focused environment.

The role emphasizes advising patients on disease management, coordinating care with families, and delivering high-quality home health services across our care team.

Qualifications

• Graduate of an accredited school of Practical Nursing.
• Current state license as a Licensed Practical Nurse.
• Current driver’s license.
• Current CPR certification.
• Negative TB screen and hepatitis consent/declination.
• Two years of experience as an LPN in a clinical care setting, home health preferred.

Responsibilities

• Support patient recovery and enable staying at home to spend time with loved ones and enjoy activities.
• Educate patients and families to reach healthcare goals through disease management.
