Job summary

CenterWell Home Health is seeking a dedicated nurse to provide one‑on‑one care in patients' homes under the supervision of the Clinical Manager. This role focuses on helping patients regain independence with personalized care plans and coordination with the healthcare team.

Requirements include an active nursing license in the practicing state, valid driver's license with auto insurance, CPR certification, and at least two years of LPN/LVN experience.

Qualifications

Required Qualifications

• Current nursing license in the practicing state.

Preferred Qualifications

• Experience in a Home Health or Hospice setting.

Pay and Hours

• Pay per visit: $33.00 – $47.00.
• Base salary: $53,000 – $73,000 per year.
• Scheduled weekly hours: 40.

Additional Information

• TB Statement: This role is patient facing and part of Humana’s TB screening program; TB screening is required.
• Driving Statement: Requires a valid state driver’s license, personal vehicle liability insurance in line with state minimums.
