Central Vermont Home Health & Hospice seeks a Home Health Licensed Practical Nurse (LPN) to join our patient-focused team. You will monitor patients in their homes, coordinate with the supervising RN, and deliver high-quality home health care to diverse individuals.

The position emphasizes autonomous practice under RN guidance, comprehensive documentation, and on-call responsiveness, with a commitment to compassionate, holistic care for the Vermont community.

Qualifications

• Valid Vermont LPN License.
• Minimum of one year progressive clinical experience; home health and wound care experience preferred.
• Valid Vermont Driver’s License.

Responsibilities

• Determine patient care needs and develop goal-oriented care plans in consultation with management/RN.
• Ensure accurate and timely documentation of patient information, adhering to HIPAA regulations.
• Work autonomously under RN direction and utilize supervisory support when necessary.
• Empower clients to explore care options and alternative plans to achieve desired outcomes.
• Participate in on-call rotation and respond to calls promptly.

Who We're Seeking

We're seeking a Licensed Practical Nurse (LPN) to join our Home Health teams. Your role involves monitoring and reporting on patients' medical and emotional well-being. Additionally, you'll communicate with the overseeing RN to coordinate support from the home health team. This position focuses on delivering care to individuals in their homes.
