Job summary

Central Vermont Home Health & Hospice seeks a Home Health Licensed Practical Nurse (LPN) to join our patient-focused team. You will monitor patients in their homes, coordinate with the supervising RN, and deliver high-quality home health care to diverse individuals.

Qualifications

  • Valid Vermont LPN License.
  • Minimum of one year progressive clinical experience; home health and wound care experience preferred.
  • Valid Vermont Driver’s License.

  • Ensure accurate and timely documentation of patient information, adhering to HIPAA regulations.
  • Work autonomously under RN direction and utilize supervisory support when necessary.

Who We're Seeking

We're seeking a Licensed Practical Nurse (LPN) to join our Home Health teams. Your role involves monitoring and reporting on patients' medical and emotional well-being. Additionally, you'll communicate with the overseeing RN to coordinate support from the home health team. This position focuses on delivering care to individuals in their homes.
