Qualifications

• Healthcare experience, preferably in referrals/intake in a home health environment.
• A nursing background is preferred but not required.
• Demonstrates good communication, negotiation, and public relations skills.
• Demonstrates autonomy, assertiveness, flexibility and cooperation in performing job responsibilities.

Responsibilities

• Directs all daily patient referral and intake operations including oversight of intake policies.
• Ensures compliance with state, federal, and Joint Commission requirements for referral/intake.
• Oversees admission in accordance with policy and maintains relationships with referral sources.
• Ensures seamless transition to home care by coordinating education, plan of care initiation, and coordination with service providers.
• Oversees insurance verification and authorization processes for maximum third-party reimbursement.
• Monitors community and customer perceptions of Comfort Home Health as a high-quality provider.
• Gathers, collates, and reports referral statistics and trends.
• Maintains knowledge of contracts and community resources to assist referral sources.
