Job summary

Qualifications

• Associates or Diploma in Nursing required.
• BSN in Nursing preferable.
• Minimum of five years in nursing with at least three in home health care.
• Registered Nurse with Connecticut license or eligibility.
• Knowledge of insurances, managed care, PDGM, OASIS, and home care eligibility.

Responsibilities

• Receives referrals from multiple sources and ensures appropriate home care service.
• Interprets referrals for accuracy and safety.
• Coordinates admissions scheduling with managers.
• Enters referrals into the EMR system.
• Verifies insurance information with clinical support.
• Prepares monthly referral activity reports when required.
• Maintains strong relationships with internal staff and outside sources.
• Provides excellent customer service and professional conduct.
• Attends required meetings and completes annual requirements.
