Job summary

Prisma Health is seeking a Nursing Liaison to coordinate referrals and intake for patients referred from hospitals and clinics. You will act as a liaison between hospitals, medical offices, home health agencies, and medical suppliers to ensure smooth patient progression to home care services.

The role requires an active nursing license and at least one year of experience in home care, hospice, or post-acute settings.

Qualifications

• Graduate of an accredited school of nursing.
• One year of experience in home care, hospice or post-acute care.
• Holds a current LPN or RN compact/multistate license recognized by the NCSBN Compact State or licensed to practice in the state of work.

Responsibilities

• Coordinates internal referrals and intake processing for referred patients.
• Acts as liaison between hospitals, medical offices, home health agencies, and medical suppliers to ensure seamless transitions to home care services.
• Educates patients and families about admission criteria and homecare options.
• Maintains ongoing communication with hospital case management and other care providers to ensure continuity of care.
• Performs QA audits as requested and supports nursing staff and office operations.
