Job summary

Mercy seeks an RN Intake professional to manage Hospice, Home Health and Home Infusion referrals across acute care and clinic settings. You will be the main liaison with discharge planners, social workers and physicians, ensuring timely discharges and continuity of care within Mercy Health System.

Qualified candidates hold an RN license, with at least three years of nursing experience; Home Health background is preferred and inpatient acute experience is valued.

Qualifications

• RN license in applicable state required.
• Minimum three years of nursing experience.
• Home Health experience preferred; inpatient acute setting preferred.

Responsibilities

• Accept Hospice, Home Health and Home Infusion referrals in acute care, physician clinic and selected venues.
• Maintain active presence in these settings and deliver excellent customer service to discharge planners, social workers, physicians and patients.
• Assist in timely discharges and support continuity of care within Mercy Health System.
