Choice Health at Home in Tyler, TX is seeking a Home Health Intake Coordinator to manage the initial patient referrals and document them promptly. You will verify payor sources, obtain authorizations, and enter data into the EMR system to ensure a smooth start of care.

You will collaborate with Nursing Supervisors, patients, families, and the interdisciplinary team to coordinate care and respond to needs with professionalism.

Qualifications

• Minimum one year of experience in referral intake is preferred.
• Experience with third party payers, Medicare and Medicaid highly desirable.
• Medical field background preferred, including familiarity with medical terminology.
• Excellent computer and customer service skills.
• LVN/LPN preferred
• Valid Driver’s License
• Valid Auto Insurance
• HomeCare HomeBase experience preferred

Responsabilités

• Receives and documents new referrals in a timely manner.
• Verifies each patient’s payor source and maintains insurance authorizations and initial certifications.
• Coordinates care with Nursing Supervisors, patients, and the interdisciplinary group.
• Provides effective communication to patients, staff, other health care professionals and referral sources.
• Responds to agency and patient needs in a professional and creative manner.
• Participates in Agency-sponsored in-service training.
• Participates in the Agency’s QAPI program.
