Job summary

Pathways Healthcare in Concord, NH is seeking a full-time healthcare sales professional to identify and admit eligible patients into our home health and hospice programs. You will collaborate with home care teams, patients, families, and providers to ensure coordinated plans of care, while building strong hospital and discharge-planner relationships.

The role offers a competitive salary range of $65,000 to $80,000 with bonus potential, mileage reimbursement, and a comprehensive benefits package.

Qualifications

• Healthcare sales experience required.
• Prior home health, hospice, long-term care, or SNF exposure preferred.
• Strong communication, negotiation, and presentation skills.
• Ability to manage quotas with minimal supervision.
• Experience with care coordination and hospital relationships a plus.

Responsibilities

• Identify, evaluate, and admit eligible patients into home health and hospice programs.
• Coordinate conversations with home care teams, patients, and families.
• Interpret clinical documentation and assess hospice eligibility with providers.
• Communicate with physicians, nurses, and social workers to establish care plans.
• Travel within designated service area to meet performance goals.
