Lehigh Valley Health Network is seeking a Registered Nurse to join our Hospice team as a Case Manager in East Stroudsburg. This full-time day-shift role coordinates home-based care, leads an interdisciplinary team, and ensures care plans progress toward patient goals and quality outcomes.

Responsibilities include plan of care development, collaboration with physicians, education for patients and families, and CMS data collection. Travel to patient sites is required; Epic EMR experience preferred.

Qualifikationen

• Specialized Diploma NLN approved/accredited nursing program.
• 1 year recent experience providing direct care in a healthcare setting.
• Ability to travel to patient care assignments.
• Knowledge of CMS/Regulatory data collection.
• Ability to work with interdisciplinary care team.

Aufgaben

• Coordinate home care for an assigned caseload with the interdisciplinary team.
• Develop and revise plans of care with physicians and team.
• Ensure timely communication of patient status changes to the team.
• Educate patients and caregivers on care plans and end-of-life options.
• Manage data collection for CMS and ensure compliant care delivery.
