Job summary

The Clinical Field Staff Supervisor at Uniting Holding oversees the coordination of home health services for Medicare and non-Medicare patients. The role ensures quality care, adheres to clinical and agency standards, and acts as the liaison among the community, referral sources, physicians, patients, caregivers, and staff.

Responsibilities include HIPAA compliance, ongoing staff education, and active utilization of Homecare Homebase (HCHB) EHR.

Qualifications

• Must be licensed RN through the state Board of Nursing.
• At least an Associate’s Degree in Nursing.
• Prefer 1 year clinical healthcare experience and 1 year in home health; knowledge of Medicare guidelines.
• Prefer supervisory experience.
• Intermediate computer skills.

Responsibilities

• Coordinate home health services for Medicare and non-Medicare patients.
• Ensure quality care and compliance with agency policies and laws.
• Serve as liaison among community, referral sources, physicians, patients, caregivers and staff.
• Ensure documentation, training, and ongoing education in agency processes and EHR.
• Support adherence to regulatory timelines and plan of care development.
