Job summary

Constant Care Health Services LLC is seeking a Certified Occupational Therapy Assistant (COTA) to join our home health team. You will assist licensed OTs in developing and implementing treatment plans to improve patients' functional abilities and independence.

You will conduct assessments, design activities, collaborate with physicians, therapists, and families, and educate patients and caregivers on OT strategies. You will maintain accurate records of plans, progress, and outcomes.

Qualifications

• Certified Occupational Therapy Assistant (COTA) credential.

Responsibilities

• Assist licensed occupational therapists in developing and implementing treatment plans to improve patients' functional abilities and independence
• Conduct occupational therapy assessments and evaluations to identify patients' needs and goals
• Design and implement meaningful and engaging occupational therapy activities and interventions to promote patients' physical, cognitive, and emotional well-being
• Collaborate with interdisciplinary teams, including physicians, therapists, and family members, to ensure comprehensive care and coordination
• Provide education and training to patients, family members, and caregivers on occupational therapy techniques and strategies
• Maintain accurate and detailed records of patient treatment plans, progress, and outcomes
