Job summary

Heritage Home Health is seeking a Certified Occupational Therapy Assistant (COTA) to provide skilled OT services in patients' homes under the supervision of a licensed Occupational Therapist. You will help patients improve independence in ADLs and functional abilities.

This on-site, field-based role emphasizes compassionate, patient-centered care. Ohio OT license required; home health experience preferred but not required; strong communication, organization, and time management are essential.

Qualifications

• Graduate of an accredited Occupational Therapy Assistant program.
• Current Ohio OTA license required.
• Home health experience preferred but not required.
• Strong communication, organizational, and time management skills.

Responsibilities

• Provide skilled occupational therapy services per plan of care under supervision of licensed OT.
• Assist patients with activities of daily living to improve independence.
• Improve strength, coordination, endurance, and fine motor skills.
• Educate patients and families on safety and adaptive techniques for home use.
• Document visits and progress in the EHR system accurately and timely.
• Collaborate with OT, physicians, nurses, and care team members.
