Enhabit Home Health & Hospice is seeking a driven business development professional to manage referral source relationships and drive post-acute care transitions. You will educate and collaborate with physicians, hospitals and community partners to grow referrals and ensure patient-centered care.

The role emphasizes in-person collaboration, knowledge of Medicare home health and hospice operations, and a strong commitment to quality outcomes and patient satisfaction.

Qualifikationen

• Bachelor's degree or equivalent in healthcare administration, marketing, nursing, therapy or social work.
• Experience in healthcare business development, community relations, marketing or sales preferred.
• Knowledge of federal, state, and local laws governing Medicare home health and hospice operations.
• Requires valid state driver license and automobile liability insurance.
• Strong communication, presentation and interpersonal skills.
• Clinical background or license preferred for clinical roles.

Aufgaben

• Serve as the primary relationship manager between Enhabit and referral sources to support post-acute care transitions and improve patient outcomes.
• Educate referral sources, patients, families and community partners about Enhabit services and program requirements to grow referrals.
• Maintain knowledge of regulatory guidelines for Medicare certified home health and hospice operations.
• Build and maintain relationships with physicians, hospitals and senior living communities to drive referral growth.
