Qualifikationen

• CNA certification required in state you work.
• High School Diploma or equivalent.
• Minimum one year healthcare experience.
• Driver’s license and ability to drive ~20% of workday.

• Provide compassionate, patient-centered care in the home/hospice setting.
• Assist with activities of daily living (ADLs).
• Educate patients and caregivers on care plans and routines.
• Respond promptly to patient needs and document in EMR.
• Work independently while staying connected with care team.
