Job summary

Residential Home Health and Residential Hospice is seeking a part-time Certified Nursing Assistant to provide in-home care for patients in the Camp Hill, PA area. You will perform ADLs, observe patient conditions, document in the EMR, and coordinate with an interdisciplinary home health team to ensure comfort and quality of life.

Travel within the territory is required. The role offers autonomy, supportive leadership, and opportunities for career growth within a nationally recognized home health

Qualifications

• Active Certified Nursing Assistant (CNA) certificate in the state you work
• High School Diploma or certificate
• Minimum one year healthcare experience
• Valid driver’s license and ability to spend ~20% of workday driving to patient locations

Responsibilities

• Deliver compassionate, patient-centered care in a home health/hospice setting
• Assist with activities of daily living and observe/report changes in condition
• Educate patients and caregivers on care plans and daily routines
• Maintain accurate, timely documentation in the EMR and communicate with the care team
• Travel to patient homes within assigned territory

What You’ll Do:

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life

• Provide hands-on care in the home, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support patient safety and independence

• Educate and support patients and caregivers by reinforcing care plans, promoting healthy routines, and encouraging confidence with daily care tasks

• Respond promptly to patient needs, communicate effectively with the care team, and maintain accurate, timely documentation in the EMR

• Work independently, but never alone

• Travel to patient homes within assigned territory
