Job summary

Safe Life Home Health seeks dedicated CNAs to join our team in Kane County, IL. As a Home Health Aide/CNA, you will assist patients with daily activities, monitor vital signs, and support overall well-being in a home health setting.

You will use EMR tools like Kinnser by WellSky, document care, and collaborate with nurses and therapists to ensure compassionate, person-centered care. A valid CNA certificate and CPR are required.

Qualifications

• Active CNA certificate.
• Valid driver's license and car insurance.
• CPR certification.
• Experience in home health is a plus.
• Familiarity with EMR Kinnser by WellSky is advantageous.
• Excellent communication skills.

Responsibilities

• Assist with Activities of Daily Living (ADLs): bathing, dressing, grooming, and toileting for home health patients.
• Mobility support: assist with walking, transferring, and use of mobility aids.
• Vital sign monitoring: take and record BP, pulse, and temperature.
• Medication reminders and ensuring schedules are followed.
• Nutrition assistance: meal prep, feeding, and dietary monitoring as prescribed.
• Hygiene and comfort: maintain hygiene and comfort, change bed linens, oral care.
• Observation and reporting: note changes in condition to the nursing team.
• Documentation: record care provided and vital signs in the EMR (Kinnser by WellSky).
• Communication: maintain clear communication with patient, family, and healthcare team.
• Safety measures: follow infection control and safety protocols.
• Transportation: provide transport to appointments or assist with mobility.
• Emotional support: offer companionship and emotional support.
• Collaboration: work with nurses and therapists for comprehensive care.
• Patient education: educate patients and families on self-care and safety.
