Job summary

Safe Life Home Health Care, LLC is seeking a part-time Home Health Aide / CNA in Bartlett, IL to provide compassionate care to home health patients. You will assist with daily activities, support mobility, monitor vital signs, and collaborate with the nursing team to ensure patient well-being.

Responsibilities include medication reminders, documentation in EMR (Kinnser by WellSky), infection control, and offering emotional support and education to patients and families.

Qualifications

Active CNA certificate and CPR certification.
Valid driver's license and car insurance.
Excellent communication and compassionate patient care.
Prior home health experience is a plus.
Familiarity with EMR Kinnser by WellSky advantageous.
Familiarity with infection control and safety protocols.

Responsibilities

Assist patients with activities of daily living (ADLs).
Support mobility and transfers using aids.
Monitor vital signs and document findings.
Provide medication reminders and schedule adherence.
Prepare meals and monitor dietary restrictions.
Maintain patient hygiene and comfort.
