CareAparent in Woodbury, MN seeks a Clinical Supervisor to lead the nursing team, drive daily clinical execution, and ensure care delivery meets regulatory and company standards.

You will supervise RNs and LPNs, manage documentation and OASIS accuracy, collaborate with intake and operations, and support on‑call coverage for patient care.

Requirements: MN RN license, 5+ years in home health with OASIS, valid driver’s license, and BLS/CPR preferred.

Qualificações

• RN license in Minnesota is required.
• Minimum 5 years of home health experience with knowledge of OASIS assessments.
• Valid driver’s license and insured vehicle.
• BLS/CPR preferred.
• BSN is preferred but not required.

Responsabilidades

• Lead, coach, and manage clinical staff to ensure consistent performance and quality care.
• Oversee charting, documentation accuracy, and CMS/audited processes.
• Review referrals to ensure scope of practice and appropriate care plans.
• Coordinate with intake, scheduling, and operations for timely admissions.
• Participate in on‑call coverage to support after‑hours decision making.
• Monitor KPIs, drive improvement, and promote data‑driven changes.
