Job summary

Enhabit Home Health & Hospice in Illinois seeks an experienced nurse leader to coordinate comprehensive home health services, ensuring care meets standards and regulatory requirements. You will lead care coordination, collaborate with clinicians, physicians, and caregivers, and uphold quality patient outcomes.

Qualifications

• Must be a graduate of an approved school of professional nursing.
• Must maintain a valid clinical license per state requirements or compact license rules.
• Experience as a licensed professional may be deferred with VP/Regional President approval.
• Minimum of two years of nursing experience within the last 36 months.
• Minimum of one year of home health experience.
• Knowledge of federal, state and local home health laws/regulations.
• CPR certification required.
• Must have a valid state driver license and automobile liability insurance.
• Basic technology skills including mobile devices.
• Preferred: management experience and bilingual (English/Spanish).
• In-office attendance is essential; remote work is not provided except for designated full-time remote positions.

Responsibilities

• Coordinate overall home health service delivery to patients according to standards and procedures.
• Review and approve patient information during start of care, recertification, resumption of care, or evaluation visits.
• Facilitate relationships between physicians, referral sources, patients, caregivers, and staff.
• Schedule patients to field clinicians and communicate with all parties to maintain care coordination and continuity.
