Job summary

Canyon Home Care and Hospice is seeking an Assistant Clinical Manager to support the Clinical Manager and Administrator in supervising home care and hospice services. The role ensures ongoing patient care at home, participates when the Clinical Manager is unavailable, and helps coordinate referrals and plan of care with physicians.

The position emphasizes adherence to regulations, staff development, and staffing optimization to deliver high-quality patient care.

Qualifications

• Must meet qualifying standards of the Clinical Manager.
• Licensed Physician, RN, or other licensed health professional as specified (Physician, RN, PT, SLP, OT, Audiologist, Social Worker).
• At least one year of supervisory experience in home care or related health program.
• Utah license required for supervision of professional services, including nursing supervision.

Responsibilities

• Understand and adhere to applicable regulations, standards, and agency policies.
• Participate in development of standards for safe, effective patient/family service.
• Supervise, guide and develop patient care personnel skills and performance.
• Assist in staffing decisions to meet objectives and maximize utilization of personnel.
• Assist in recruiting, hiring, evaluating and terminating personnel; document disciplinary action.
