Job summary

Qualifications

• Licensed professional as per state regulations (ID/OR/UT).
• Minimum required supervisory experience in home care or related health program.

Responsibilities

• Understand and adhere to all applicable regulations, standards, and agency policies.
• Supervise, guide and develop patient care personnel skills and performances.
• Assist Administrator with recruiting, hiring, evaluating and terminating personnel; document disciplinary actions.
• Plan and coordinate patient care, staffing patterns, referrals, and ensure updates to care plans with physician signatures.
• Coordinate interdisciplinary team services and ensure availability of Clinical Manager during all operating hours.
• Assist with policy development, QAPI plans, and infection control management.
