Job summary

Responsibilities

• Lead and support nursing and aide performance to ensure compliance and quality standards.
• Manage patient eligibility determinations, referral intake, and coordination of services.
• Oversee patient care coordination including scheduling, documentation, and care plan execution.
• Coordinate and ensure accurate plans of care are developed and followed.
• Support staff training and continuous improvement in clinical practice.
• Resolve care concerns and address issues that affect patient outcomes.
• May provide occasional direct patient care as needed.
