Job summary

CareRing Health in Philadelphia is seeking a clinical leader to organize and direct the home health functions for our community hub. You will oversee a team of clinicians to ensure patients receive exceptional in-home care.

Responsibilities

What You Will Do

• Organize, develop, manage, and direct the home health functions of your community health hub
• Manage a team of clinicians to ensure our patients receive exceptional care in their homes
• Assess, plan, direct, and evaluate each patient’s care plan to ensure it meets the patient’s needs
• Conduct patient visits (as needed)
• Manage emergency situations in a prompt, precise, and professional manner
• Complete medication administration and treatment needs for all assigned patients
