Job summary

Trinity Health At Home – Muskegon is seeking a Home Health Clinical Manager to oversee the clinical and operational elements of our homecare program, driving outcomes, productivity, and budget targets.

You'll supervise agency staff, coordinate care, and partner with leadership to ensure compassionate, patient‑centered service. Strong clinical background in nursing or therapy and progressive supervision are essential for success.

Qualifications

• Current RN or Clinical Therapy License in Michigan
• Bachelor’s degree in nursing or therapy; Master’s degree encouraged
• Minimum three years’ experience in a skilled homecare agency
• Progressive supervisory or leadership experience
• Valid Driver’s License with reliable transportation

Responsibilities

• Oversee clinical and operational elements of the homecare program
• Achieve targets, metrics, and budget targets
• Provide functional supervision of agency home care staff
• Coordinate care and ensure documentation and care coordination

Minimum Qualifications

• Current RN or Clinical Therapy License in the State of Michigan
• Bachelor’s degree in nursing or therapy field required, Master's degree encouraged, (or if therapy, degree required from discipline)
• Minimum three years' experience in a skilled homecare agency.
• Must possess progressive supervisory or leadership experience.
• Must have well developed interpersonal and communication skills to provide effective interface with clients, professional clinical staff and all other related team members.
• Must possess a valid Driver’s License and have reliable transportation to and from the work site
