Job summary

Talent Solutions Partners is seeking a Clinical Manager in San Diego to oversee patient care services and staff. The role coordinates care plans with an interdisciplinary team, ensures safe delivery of services, and drives QAPI initiatives across the organization.

The position requires RN licensure in the operating state, ongoing leadership, and the ability to travel between locations as needed. Strong knowledge of CHAP standards and survey readiness is essential.

Qualifications

• RN with active license in the state of operation.
• Minimum 5 years RN experience with 2+ years supervision in home health.
• Experience managing RN services in home, community, or clinic settings preferred.

Responsibilities

• Oversee day-to-day clinical and administrative operations as primary staff contact.
• Ensure compliance with professional standards, policies, and state/federal regulations.
• Provide leadership and backup coverage for PCM and CSS roles as needed.
• Coordinate intake and referrals; develop and update individualized care plans with the care team.
• Drive QAPI initiatives, track incidents, analyze trends, and implement corrective actions.
• Maintain CHAP/regulatory survey readiness and oversee HHCAHPS survey distribution.
• Lead Emergency Preparedness Planning (EPP) and post-activation reporting.
