Job summary

AdventHealth in Dade City, FL seeks an experienced Home Health Lead to plan, coordinate, and direct clinical activities for our home health program. You will guide the assessment, planning, implementation, and evaluation of patient care, ensuring regulatory compliance and optimal outcomes.

The role requires RN licensure, 1+ year supervisory experience in Medicare-certified home care, and strong knowledge of OASIS and EMR systems. On-site schedule, Mon-Fri, with potential on-call responsibilities.

Qualifications

• Must be an RN with an active license and state requirements.
• Minimum 1 year of supervisory or administrative experience in Medicare-certified home care.
• Experience in home health and OASIS assessment preferred.

Responsibilities

• Plans, coordinates, manages, and directs all clinical activities and programs for home health in accordance with regulatory guidelines and professional standards.
• Provides direction in the assessment, planning, implementation, and evaluation of patient care.
• Oversees staffing, scheduling, productivity, and compliance with care plans; drives quality improvement and billing accuracy.
