Comforting, Loving & Helping Hands Agency, LLC seeks a part-time Clinical Manager in Florida to lead home health operations. You will supervise clinical staff, ensure regulatory compliance, and coordinate in-home care for geriatric patients, with emphasis on quality and policy adherence.

Responsibilities include case supervision, staff evaluations, ongoing education, and collaboration with organizational personnel to optimize home visits and patient outcomes.

Formación

• Florida Licensed RN with at least two years of home health experience.
• Licensed Physical Therapist with at least two years of home health experience.
• Licensed Social Worker with at least two years of home health experience.
• Licensed Occupational Therapist with at least two years of home health experience.
• Licensed Speech Therapist with at least two years of home health experience.
• Licensed Physician with at least two years of home health experience.
• Minimum management/supervisory experience 2-3 years.
• Demonstrate knowledge to care for geriatric population primarily.
• Proficient with Oasis and Medicare Compliance
• Medicare Home Health experience 3-5 years (preferred)
• Demonstrate organizational and leadership skills.

Responsabilidades

• Receives case referrals and determines home care needs.
• Reviews and evaluates cases to guide clinicians for quality home care.
• Assists in establishing goals, priorities, and care plans.
• Attends case conferences to coordinate care.
• Assists in interviewing and onboarding new personnel.
• Orientates new staff to enhance success of clinicians.
• Assesses clinicians for policy and procedure compliance.
• Participates in public relations and community activities.
• Provides monthly SHP scorecards and ongoing education to clinicians.
• Identifies challenges in documentation and assists clinicians.
• Strives for 24-hour admission following referral.
• Resolves barriers to patient admission.
• Supports professional development and clinical excellence.
• Performs field visits to mentor clinicians.
• Conducts formal clinical evaluations for discharge or recertification.
