Residential Home Health and Hospice is hiring a Certified Nursing Assistant for a Hospice position serving patients in their homes in Wheeling, IL. Eligible for a $5,000 sign-on bonus. You will deliver compassionate, patient-centered care, assist with ADLs, and support families throughout the hospice journey.

Responsibilities include documenting care in the EMR, traveling to patient homes within the territory, and collaborating with an interdisciplinary team to ensure comfort and quality of life.

Formación

• Active Certified Nursing Assistant (CNA) certificate in the state you work.
• High School Diploma or equivalent.
• Minimum one year healthcare experience; Hospice experience preferred.
• A valid driver’s license and ability to travel up to ~20% of workday.

• Travel to patient homes within assigned territory.

• Support and guide patients and caregivers by reinforcing the care plan, offering emotional support, and helping them understand what to expect during the hospice journey
• Respond promptly to patient needs with empathy and attentiveness and accurately document care provided in the EMR
• Work independently, but never alone
• Travel to patient homes within assigned territory

CNA Qualifications

• Active Certified Nursing Assistant (CNA) certificate in the state you work
• High School Diploma or certificate
• Minimum one year healthcare experience, Hospice experience preferred
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
