Job summary

Home & Health Care Management is seeking a Social Worker to join the Care Management Team in Chico to coordinate in-home support for older adults and clients with disabilities in Butte County.

You will work with a Master’s or Bachelor’s in Social Work, have strong computer literacy, and drive your own vehicle. The role runs Monday–Friday, 8am–5pm, with no weekend work, and requires CPR within 30 days of hire.

Qualifications

• Master's or Bachelor's degree in Social Work or related field.
• Valid California Driver’s License with clean DMV report.
• CPR certificate within 30 days of hire.

Responsibilities

• Assist team with locating community resources (housing, equipment, services) to meet participant needs.
• Advocate to ensure timely medical services and safe community participation.
• Respond to inquiries from CMT, participants and others for smooth care coordination.
• Educate participants on benefits of services and community supports.
• Assist with Medi-Cal, IHSS, CalFresh and Regional Center applications; arrange transportation.
