Job summary

Home & Health Care Management in Redding, CA, seeks a Social Worker (MSW/BSW) to join the Care Management Team. You will collaborate with a nurse and SW to provide comprehensive in-home case management for older adults, children, and individuals with disabilities.

You bring a Master’s or Bachelor’s in Social Work, strong problem‑solving skills, and computer literacy; experience with EMR is preferred. The role is full‑time at our Redding office, with 8am–5pm weekdays and no weekends.

Qualifications

• Master's or Bachelor’s degree in Social Work or related field.
• Skilled in operating a personal computer with standard software.
• CPR certificate or equivalent certification within 30 days of hire.

Responsibilities

• Assists the team with finding community resources (housing, medical equipment, primary care, essentials, transportation, caregivers).
• Engages in advocacy to ensure timely medical services and safety in the community.
• Responds to questions from CMT, participants, and other entities for care coordination.
• Provides education to participants about services and community supports.
• Assists participants with applications (Medi-Cal; IHSS; CalFresh; Regional Center) and coordinates transportation.
• Documents all consumer interactions and maintains timely reporting and documentation requirements.
• Observes participants and reports changes; conducts daily case conference with CMT.
• Performs other related duties as assigned.
