Job summary

Lakeview Village Home Health in Kansas seeks a Registered Nurse Case Manager to deliver comprehensive nursing care at clients' homes, lead Home Health Aides and staff nurses, and ensure care is properly implemented.

You will perform ICD-10 coding and OASIS assessments, document thoroughly, and promote health, early detection and restoration. Requirements include an RN license in Kansas, CPR, and preferred Case Manager certification.

Qualifications

• Graduate of an accredited school of nursing.
• Current, unrestricted RN license in Kansas.
• Current CPR certification.
• Case Manager certification preferred.
• Knowledge of state, federal, and medical home health regulations.
• Prior home health or case management experience preferred.
• Reliable transportation with valid driver's license.
• Strong communication and teamwork skills.
• Ability to meet physical requirements listed in job description.

Responsibilities

• Provide comprehensive nursing services in home/community settings.
• Lead Home Health Aides and Staff Nurses to ensure proper care delivery.
• Handle ICD-10 coding for diagnoses and perform OASIS assessments.
• Conduct admissions, transfers, re-certifications, and discharges.
• Record all symptoms and progress accurately for continuity of care.

Lakeview Village Home Health provides Medicare-certified, short-term, goal-oriented medical care and support in the comfort of the client’s own home. Home Health services are provided to residents of Lakeview Village, as well as to seniors living in Johnson, Wyandotte, and Douglas counties.We are seeking a Registered Nurse Case Manager who will:Provide comprehensive nursing services to individuals and their families in a community setting, with emphases on the promotion and maintenance of health, early detection, and restoration measuresProvide leadership for Home Health Aides and Staff Nurses to ensure that care is properly and effectively implemented.Demonstrate a full understanding and effective application of ICD-10 coding for primary and secondary diagnoses.Demonstrate a full understanding and effective application of OASIS assessments.
