Lakeview Village Home Health in Lenexa, KS is seeking a Registered Nurse Case Manager to deliver comprehensive nursing care in clients' homes and to support a care team of aides and nurses. The role emphasizes health promotion, early detection, and restoration, with leadership duties for frontline staff.

The ideal candidate holds an RN license in Kansas, CPR certification, and preferred Case Manager credentials.

Qualifications

• Graduate of an accredited school of nursing.
• Unrestricted RN license in Kansas.
• Current CPR certification.
• Case Manager certification (preferred).
• Home health and/or case management experience preferred.
• Reliable transportation and a valid driver's license.
• Excellent communication skills and empathy for the elderly.

Responsabilités

• Provide comprehensive nursing services in a community/home setting.

• Perform OASIS assessments and admissions/transfers/re-certifications.

Job Details: Job Location: Lakeview Village - LENEXA, KS 66215, Position Type: Full Time, Education Level: RN, Job Shift: Day, Job Category: Health Care, Lakeview Village Home Health provides Medicare-certified, short-term, goal-oriented medical care and support in the comfort of the client’s own home. Home Health services are provided to residents of Lakeview Village, as well as to seniors living in Johnson, Wyandotte, and Douglas counties.

We are seeking a Registered Nurse Case Manager who will:

• Provide comprehensive nursing services to individuals and their families in a community setting, with emphases on the promotion and maintenance of health, early detection, and restoration measures
• Provide leadership for Home Health Aides and Staff Nurses to ensure that care is properly and effectively implemented.
• Demonstrate a full understanding and effective application of ICD-10 coding for primary and secondary diagnoses.
• Demonstrate a full understanding and effective application of OASIS assessments. Performs OASIS admissions, transfers, re-certifications, resumptions of care, and discharges.
• Thoroughly and completely record all symptoms, progress, and treatments in the appropriate charting system to ensure the continuity of care, maintenance of accurate legal documents, and caring, professional services.

Qualifications: Ideal candidates will have the following qualifications and skills:

• Graduate of an accredited school of nursing
• Possess a current, unrestricted RN state nursing license in the state of Kansas
• Current CPR certification
• Case Manager certification (preferred)
• Understanding and effective application of state, federal, and medical regulations as they apply to home health
• Prior home health and/or case management experience (preferred)
• Reliable transportation and a valid state driver’s license
• Excellent communication skills
• Possesses and empathetic approach to the elderly
• Able to work in a team setting with residents, relatives, staff, and others
• Able to meet the physical demands of the job: Exerts up to 50 lbs. of force occasionally, and/or up to 20 lbs. of force frequently, and/or up to 10 lbs. of force constantly to move objects; must have close visual acuity to perform an activity at distances close to the eyes such as: preparing and analyzing data; viewing a computer terminal; extensive reading. In addition, the visual acuity to determine the accuracy, neatness, and thoroughness of the work or to make general observations of environment and/or resident/customer is required; must be able to hear sounds at normal speaking levels and express ideas by means of talking; stoops by bending the spine or at the waist; kneels by bending legs at knee to come to a rest on knee or knees; and crouches by bending the leg and spine; reaches by extending hand(s) and arm(s) in any direction; stands for sustained periods of time, walks from one site to another; pushes, pulls, and lifts using upper extremities and back muscles; uses fingers for movements such as picking, pinching, typing; grasping with the fingers and palm and feeling attributes of objects by touching with skin, particularly that of fingertips; repetitively moves wrists, hands, and/or fingers.
