The ideal candidate holds an RN license in Kansas, CPR certification, and preferred Case Manager credentials.

Qualifications

• Graduate of an accredited school of nursing.
• Unrestricted RN license in Kansas.
• Current CPR certification.
• Case Manager certification (preferred).
• Home health and/or case management experience preferred.
• Reliable transportation and a valid driver's license.
• Excellent communication skills and empathy for the elderly.

Responsibilities

• Provide comprehensive nursing services in a community/home setting.
• Lead Home Health Aides and Staff Nurses to ensure proper care.
• Apply ICD-10 coding for primary and secondary diagnoses.
• Perform OASIS assessments and admissions/transfers/re-certifications.
• Document symptoms, progress, and treatments accurately in the charting system.
