AdventHealth seeks a Registered Nurse for home health in Shawnee Mission, KS, evaluating the home environment for safety and coordinating with the healthcare team to deliver direct nursing care and care planning.

The role involves safety assessments, patient history review, and implementing care plans with stakeholders, using health coaching to engage families. RN licensure and BLS required.

Qualifications

• Must be a Registered Nurse (RN).
• Driver's License is required.
• Basic Life Support (BLS) certification required.
• COSC certification preferred.
• 1+ relevant clinical nursing experience required.
• Home health experience with Medicare as case manager preferred.

Responsibilities

• Evaluates the home environment for safety, infection control, and community resource needs.
• Reviews patient history, physical diagnostics, and laboratory data, and reports abnormal results to the physician.
• Implements care plans through direct patient care, coordination, delegation, and supervision of healthcare team activities.
• Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments in various home situations.
• Uses motivational interviewing and health coaching techniques to engage stakeholders in care management.
