AdventHealth in Kansas is seeking a Registered Nurse to evaluate home environments for safety and infection control, review patient histories, and implement care plans through direct patient care and coordination with the healthcare team.

The role requires an RN license, an ADN and preferably a BSN, and at least 1+ year of relevant home health case management; strong computer skills and the ability to work independently are essential.

• Ability to delegate tasks to appropriate personnel as indicated by skill level and professional standing.
• Strong computer and technology skills.

Responsabilités

• Evaluates the home environment for safety, infection control, and community resource needs.
• Reviews patient history, physical diagnostics, and laboratory data, and reports abnormal results to the physician.
• Implements care plans through direct patient care, coordination, delegation, and supervision of healthcare team activities.
• Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments in various home situations.
• Uses motivational interviewing and health coaching techniques to engage stakeholders in care management.
