Comprehensive Home Health Solutions (CHHS)– Reno, Nevada is seeking an experienced Client Care Coordinator to join our Home Care team. This role supports, coaches, and develops caregivers while ensuring high-quality care for older adults and families.

The position emphasizes relationship-building with clients and families, coordinating care with scheduling and intake, and maintaining thorough documentation.

Qualifications

• Experience in home care, caregiving, healthcare coordination, or senior care.
• Strong communication and interpersonal skills.
• Highly organized with the ability to manage multiple priorities.
• Comfortable coaching, training, and mentoring caregivers.
• Valid driver’s license and reliable transportation.
• Willing to travel to client homes throughout the service area.

Responsibilities

• Develop, maintain, and update individualized client care plans.
• Build relationships with clients and families and address questions, concerns, and changing care needs.
• Coordinate with caregivers, Scheduling, Intake, and the Lead Client Care Coordinator to ensure continuity of care.
• Conduct required in-home caregiver supervisory visits and regular client/caregiver check-ins.
• Monitor caregiver performance, professionalism, safety, and adherence to care plans.
• Participate in caregiver onboarding, orientation, training, coaching, mentoring, and competency validation.
• Identify training needs and provide or coordinate additional education.
• Respond promptly to client and caregiver concerns and elevate significant issues appropriately.
• Participate in daily morning huddles when not engaged in a client visit or other time-sensitive responsibility.
• Assist with admissions, staffing transitions, schedule changes, and other operational needs.
• Maintain accurate and timely care coordination, supervisory, and training documentation.
