What You Can Expect

Support both Home Health and Hospice
Complete psychosocial assessments and plans of care for home health patients.
Identify and provide interventions and goals as indicated by the plan of care.
Consult with physicians, healthcare professionals, the home health care team, and community partners.
Be part of the multi-disciplinary team.
Provide community resource information to home health patients/families.
Compensation: Paid hourly including drive time and mileage reimbursement for traveling or fleet car.
Sign‑on bonus eligible.
Professional development: Clinical supervision and in‑house educational opportunities.
Schedule: Monday‑Friday.

Qualifications

Education: Masters degree.
Licences/Certifications: Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) in states of practice.
