Job summary

CenterWell Home Health, a Humana company, is seeking a Home Health Aide to support patients at home under the supervision of an RN. You will assist with medications, vitals, hygiene, feeding, mobility, and daily activities while documenting care and ensuring safety.

The role requires recent HHA/CNA training and valid state certification. We offer comprehensive benefits including medical, dental, vision, 401(k), and paid time off.

Qualifications

• Must meet applicable state certification requirements.
• Complete Certified Nursing Assistant or Certified Home Health Aide Program within the last 24 months.
• Must be in good standing on the HHA Registry (if applicable) and have completed HHA/CNA course to work for a Medicare certified agency.

Responsibilities

• Provide direct patient care to patient under the direction of the RN and according to the Aide Plan of Care (POC).
• Assist in feeding patients and communicate basic principles of nutrition; observe and record intake.
• Safely position patient for meals and assist with self-feeding.
• Assist with patient toileting and catheter care.
• Provide skills to safely assist with mobility, transfers and ambulation per Plan of Care.
• Document interactions with patients and care team for continuity of care.
• Practice infection control and maintain a clean, safe environment.
