Humana Inc., CenterWell Home Health is seeking a Home Health Aide to deliver in-home care under RN supervision. You will follow the Plan of Care to support rehabilitation, mobility, and daily activities for patients in their homes.

Responsibilities include vital signs, medication observation, personal care, feeding assistance, mobility support, documentation, and adherence to infection control. A valid driver’s license and HHA/CNA certification are required.

Formación

• Complete Certified Nursing Assistant or Certified Home Health Aide Program within the last 24 months.
• Must meet applicable state certification requirements.
• Must have a valid driver’s license and reliable transportation.
• Must be in good standing on the HHA Registry (if applicable).

Responsabilidades

• Provide direct patient care under RN direction and Plan of Care.
• Assist with self-administered medications and observe dosing.
• Take and report vital signs (temp, pulse, BP).
• Assist with bathing, grooming, hygiene and personal care.
• Support feeding and nutrition monitoring.
• Position for meals and assist with self-feeding.
• Assist with toileting and ostomy care.
• Support mobility, transfers and ambulation per Plan of Care.
• Document care provided and changes for continuity of care.
• Maintain infection control and a clean, safe environment.
• Assist with light household tasks as needed.
• Collaborate with caregivers, doctors and staff.

Conocimientos
