CenterWell Home Health is seeking a dedicated Home Health Aide (HHA) to join our caring team. You will provide direct patient care at patients’ homes under the supervision of a Registered Nurse and according to the Plan of Care, assisting with daily activities and rehabilitation to promote independence and safety.

Applicants should have a current Certified Nursing Assistant or Certified Home Health Aide program, state certification, a valid driver's license, and at least one year of HHA/CNA

Formación

• Complete Certified Nursing Assistant (CNA) or Certified Home Health Aide (CHHA) program within the last 24 months.
• Must meet applicable state certification requirements.
• Must have a valid driver’s license, auto insurance and reliable transportation.
• Must be in good standing on the HHA Registry and have completed HHA/CNA course to work for a Medicare certified agency.
• Minimum one year experience as a Home Health Aide or CNA in past 24 months.

Responsabilidades

• Provide direct patient care under RN direction and according to the Plan of Care.
• Assist with self-administered medications and verify proper dosing.
• Take vital signs and report abnormal results to supervisor.
• Assist with personal hygiene, grooming and oral care.
• Assist with feeding and monitor nutrition and fluids.
• Assist with toileting and ostomy care as applicable.
• Support mobility, positioning, transfers and ambulation per Plan of Care.
• Document care provided and communicate with patients, caregivers and staff.
• Maintain infection control best practices and safe home environment.
• Assist with light household tasks when needed and allowed.
