CenterWell Home Health is hiring a Home Health Aide to support patients in their homes under RN supervision. You will follow the Aide Plan of Care to provide direct care, assist with medications, monitor vital signs, and support daily living activities.

You will help with mobility, hygiene, nutrition, and safe transfers, documenting care and communicating with the care team. A valid CNA/HHA program and state certification are required, along with a valid driver’s license.

Qualifications

• Must complete Certified Nursing Assistant (CNA) or Certified Home Health Aide (HHA) program within the last 24 months.
• Must meet applicable state certification requirements for HHA/CNA work.
• Valid driver’s license with reliable transportation and auto insurance.

Responsabilités

• Provide direct patient care under RN guidance and according to the Aide Plan of Care.
• Assist with self-administered medications, read labels, and monitor dosing.
• Take and document vital signs (temperature, pulse, BP) and report abnormal findings.
• Support personal hygiene, bathing, grooming, skin care, and oral care.
• Assist with feeding and monitor nutrition and fluid intake.
• Position and transfer patients safely; assist with mobility and exercises per Plan of Care.
• Record care interactions and ensure continuity of care with the team.
• Maintain infection control standards and provide a clean, safe home environment.
• Assist with light household tasks if no MD order is present.
• Operate within Humana’s TB and driver-safety programs.
