Job summary

Humana Inc. through CenterWell Home Health is seeking a Home Health Aide to support patients at home, delivering rehabilitation-focused care under the supervision of an RN. You’ll assist with activities of daily living, mobility, and monitoring vitals to ensure comfort and safety.

Requirements include CNA/HHA certification within 24 months, state licensure, and a valid driver's license. The role offers benefits and a competitive base pay with full-time hours.

Qualifications

• Complete Certified Nursing Assistant or Certified Home Health Aide Program within the last 24 months
• Must meet applicable state certification requirements
• A valid driver's license, auto insurance, and reliable transportation are required
• Must be in good standing on the HHA Registry (if applicable) and have completed HHA/CNA course to work for a Medicare certified agency.
• Minimum one year experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, home health/hospice agency.

Responsibilities

• Provide direct patient care to patient under the direction of the RN and according to the Aide Plan of Care (POC).
• Assist with medication administration by reading labels and observing proper dosing.
• Take vital signs and report abnormal results to supervisor.
• Assist with bathing, grooming, hygiene and nutrition.
• Support mobility, transfers and ambulation per Plan of Care.
