Job summary

Qualifications

• High School Diploma - Preferred.
• 1 Year - Preferred experience in home health or related field.
• Ability to perform all essential job responsibilities after three month orientation period - Required.
• Basic Life Support (BLS) - Required.

Responsibilities

• Records information accurately and legibly on appropriate forms or in computer devices according to agency policies and procedures.
• Collaborates with all personnel through patient team conference to identify, clarify and respond to the needs of the patient.
• Provides care as indicated on the Aide Assignment Sheet.
• Maintains reliable transportation and ability to travel as required.
