Base pay range

$19.50/hr - $23.10/hr

Summary

The Home Health Aide provides personal care and designated health-related services designed to maintain the client’s physical and emotional well-being.

Primary Responsibilities

1. Assist clients with treatments and/or health-related services, as delegated by licensed staff and in accordance with the service plan or plan of care.
2. Provide assistance to clients during scheduled programming, through conversation and re-orientation when appropriate, and assisting with mobility when appropriate.
3. Act as an advocate to maintain client privacy and dignity during cares.
4. Assist clients with medication administration as delegated by licensed staff and in accordance with Minnesota Home Care Licensure rules.
5. Communicate any identified changes involving client promptly to licensed staff.
6. Assist clients with personal care, including hygiene, dressing, grooming, and toileting as instructed.
7. Assist Nutritional Services staff with serving meals and enhancing the mealtime experience.
8. Maintain appropriate documentation of services as required by licensed staff.
9. Document all services provided, including client health and personal care, using specialized medical terminology.
10. Assist with homemaking activities, including general maintenance and cleanliness of the facility and apartments, and recognize safety hazards.
11. Support various other Homecare responsibilities as needed.

Lead Home Health Aide

In emergencies, may need to fill in for the Lead Home Health Aide and provide lead direction, respond to medical emergencies in the absence of licensed staff, and perform administrative tasks such as scheduling and record keeping.

Job Specifications

Education, Experience, and Credentials

• Home Health Aide certification and/or competency testing required.
• Six months of Home Health Aide or NAR experience preferred.
• Current CPR certification preferred; if not certified upon hire, must obtain within 60 days.

Knowledge, Skills, and Abilities

• Flexible and productive with interruptions.
• High customer service standards, responding promptly and thoroughly.
• Maintain confidentiality per HIPAA and compliance standards.
• Effective problem-solving skills.
• Proficient in Microsoft Word, Outlook, Internet, and database management.
• Understand and follow safety and emergency procedures.

Physical Requirements

• Ability to lift up to 50 pounds, and carry objects up to 20 pounds frequently.
• Push/pull residents in wheelchairs up to 200 pounds.
• Clear communication skills in English, both oral and written.
• Pass training modules on medication administration and documentation.
• Use office equipment and work at a computer.
• Stand, walk, and respond quickly to emergencies.
• Work flexible hours, including nights and weekends, and have reliable transportation.

• Communication devices, assistive equipment, thermometers, blood pressure monitors, laundry equipment, etc.
• Telephone, computer, and office equipment.

Work Environment

Primarily indoor, with potential exposure to blood, bodily fluids, and infectious materials. Must be adaptable to switching positions quickly to respond to residents' needs.
