Job summary

Brooklyn Center for Rehabilitation and Healthcare in Brooklyn, NY is seeking Home Health Aide Patient Escorts to support residents in a Skilled Nursing Facility. You will escort residents to appointments, monitor health, and report issues while maintaining confidentiality.

Applicants must have NYS HHA certification, be able to lift occasionally, and work well as part of a team to deliver high-quality care consistent with the Plan of Care.

DUTIES

• Escort residents to and from appointments
• Observing and monitoring Residents.
• Reporting any health issues to the supervising nurse.
• Maintain confidentiality of all Resident & Facility data.

• Valid New York State HHA Certification.
