Job summary

Spring Creek Rehabilitation & Nursing Care Center in Brooklyn is seeking a dependable Home Health Aide (HHA) to assist residents with safe transportation to clinic and dialysis appointments and provide support throughout their visits.

The role requires current NYS HHA certification; CNA certification is preferred. A high school diploma or equivalent is also valued, along with prior healthcare experience and strong communication skills.

Qualifications

• Current NYS HHA certification is required.
• CNA certification preferred.
• High school diploma or equivalent preferred.
• Previous nursing home or healthcare experience preferred.
• Strong communication and interpersonal skills.
• Professional appearance and dependable work ethic.
• Ability to prioritize resident safety and follow instructions.

Responsibilities

• Escort residents safely to and from clinic and dialysis appointments.
• Arrive approximately 1 hour before scheduled appointments.
• Assist residents with preparing for appointments and provide approved snacks for out-on-pass visits.
• Monitor residents for safety and help prevent falls or incidents.
• Report accidents, incidents, or concerns promptly to the Clinical Coordinator or ADNS.
• Ensure required medical records and paperwork accompany residents.
• Verify DNR/DNI status and required documentation.
• Communicate clinic concerns and advocate for residents.
• Coordinate assignments and appointments with the Clinic Coordinator.
• Follow all facility policies, safety procedures, and abuse-prevention regulations.
• May be required to attend multiple appointments in one day.
